Enhanced Liver Fibrosis (ELF)

Non-Alcoholic Fatty Liver Disease (NAFLD) is a very common disorder referring to a group of conditions where there is a build-up of excess fat in the liver of people who drink little or no alcohol.  Although in its early stages NAFLD doesn’t usually cause harm, it can lead to liver cirrhosis and increase your risk of diabetes, heart attack and stroke. The good news is that if it’s detected in its early stages, NAFLD can be managed through lifestyle changes and in some cases medical treatment from your Doctor.

Enhanced Liver Fibrosis (ELF)

Basic liver function tests won’t pick up signs of liver disease, particularly in its early stages. Liver fibrosis is the scarring process that represents the liver’s response to injury. Over time this can result in liver cirrhosis, liver failure and liver cancer. The main causes of fibrosis are alcohol, viral hepatitis B/C and fatty deposits within the liver due to being overweight. Although more common in people aged over 50, a recent study found that the presence of NAFLD in younger people has risen substantially over the last few decades.

The ELF blood test can help assess a quantifiable level of liver fibrosis.

Your liver performs a number of important jobs in your body, like regulating blood clotting, converting nutrients from the food you eat, helping fight infection and maintaining hormone balances.  Contrary to popular belief, liver problems aren’t just a result of excessive alcohol consumption, but they can make you very ill and even lead to death.

You can continue to eat and drink normally before your appointment, and you’ll also remain fully clothed throughout.

Which readings are included?

You will receive an ELF score which is made up from the analysis of 3 types of blood marker: HA, P111NP and TIMP-1. The results from these are then combined into a score which correlates with your level of liver fibrosis.
AST is an enzyme found throughout your body in the heart, liver, kidneys, pancreas and muscle tissues. A common measure for liver health, raised levels can reflect liver damage.
ALT is an enzyme produced by your liver and can indicate liver damage caused by alcohol, drugs or viruses (hepatitis). Raised levels could indicate inflammation in your liver or bone disorders, although it’s important to note fluctuation of ALT levels is normal over the course of the day so elevated levels do not automatically mean there is a problem.
ALP is an enzyme produced mainly in your liver and bones. Raised levels could indicate blocked bile ducts or bone disorders whereas low levels may indicate blood disorders such as anaemia. Pregnancy can also lead to raised ALP levels.
Gamma GT is an enzyme in the liver which is raised in liver and bile duct diseases. Used with ALP, it can distinguish between bone or liver problems. Gamma GT can also be elevated by large quantities of alcohol and can indicate alcohol abuse - it is raised in 75% of long-term drinkers.
Bilirubin is a breakdown product of red blood cells, which is processed by the liver and excreted via the bile duct. It can be raised in diseases of the liver or bile duct and certain blood disorders. High levels can cause yellowing of the skin and the whites of the eyes (jaundice), indicating that the liver is having trouble removing bilirubin from the blood. This can indicate liver damage.
